Please be aware that all nominations must be received by January 31. You may use this Online Nomination Form, though it is highly recommended that you e-mail the judges directly due to reliability. You can also send your nomination via postal mail. Please note that this nomination form may only be used for the regular Awards that are to be judged by the Regional Coordinator and the Vice-Regional Coordinator. If you submit a nomination with this web form, not only are copies sent to the judges, but copies are also e-mailed to the Awards Director and back to you.
